Welcome to the office of Dr. Matthew Collard!

If you are seeking to resolve an orthopedic issue you have come to the right place. Dr. Collard has helped thousands of St. Louis area adults return to an active lifestyle following a sports injury or workplace accident. He and his staff look forward to getting to know you and helping you regain the life you once knew, too.

First Steps for New Patients

To help save time at your first appointment, we ask for you to do a few things before you arrive:

step one

Download and complete our Patient History and HIPPA forms, and bring them with you to your appointment. You’ll want to review our New Patient Information and Policies as well.

step two

In addition to the completed forms, also bring:

  • Your driver’s license
  • A list of your current medications
  • Your insurance card, if applicable
  • Your co-pay, if applicable
  • A list of any questions you may have

step three

Please plan to arrive approximately 30 minutes prior to your scheduled appointment if your paperwork is completed. If your paperwork is NOT completed, please allow extra time so we can make sure everything is in order before you see Dr. Collard.
